You should absolutely run it in a windows 10 32bit environment in a vm.
I wouldn't trust some hacked ntvdm. furthermore, some windows update could come
by and either remove it or bork something.

I think running it in a 32bit vm is the smart thing to do if you want to run
doorgames. Not only that, but running it in a vm makes it easy to work with and
backup.

I run several bbses at once and what i learned is you should eliminate what can
go wrong.  Perhaps in the future we will have a better alternative for 64bit
oses.
